The information in this form is being collected by the Faculty of Medicine, Dentistry and Health Sciences, the University of Melbourne. The information you provide is being collected in order to record administrative details of the applicant, and for assessment. The information will be used by authorised staff for the purpose for which it was collected, and will be protected against unauthorised access and use. The names of successful applicants and a summary of their projects will be reported to relevant University committees and senior officers. If you do not provide all of the information requested on this form, your application may be deemed ineligible and removed from consideration by the committees.

Collection of Personal Information: The University of Calgary is collecting your personal information under the authority of Section 4(c) of the Protection of Privacy Act (SA 2024, c P-28.5). Your information will be used by the CSM Grant Development Office to assess your application to this program, including peer review, administration and evaluation of the program. Your information may be shared with relevant leadership at the Cumming School of Medicine, your department, and peer reviewers. It may also be used in an automated system to support decision-making or generate recommendations. The information will be managed in accordance with applicable security and privacy policies.
